The "Health Assessment" is available on all of our Usana websites. If you have not set up your own Usana website, you can use mine at http://gwen.usana.com Either way if it is yours or mine, just look on the left hand side of the page and you will find it. I'm re-sending you the other two documents we discussed, referring to your vision statement and to making a list.
As for the doctors' offices. There are many choices. The most comprehensive is the Product Brochure, which you can purchase via the "Shop at Usana" tab on your website. There are also various one-sheets and brochures which you can look at when you go to your back office, go to the "Prospecting" tab, then "Tools". In addition, on that page, you will find a "vendor link" to ProTools, which sells all the amazing tools we use to build our business.
